Job Summary
Under general supervision of the Program Manager, performs health education and outreach activities to encourage healthy behaviors that will help community members to restore, promote and maintain optimal health within program areas. Job Responsibilities
~ Must be capable of regular and predictable attendance at a specified location in order to perform assigned tasks.
~ Develops educational programs relative to public health and safety according to the goals and objectives of the program; identifies strategic partnerships with organizations and stakeholders that promote health education.
~ Educates on the importance and availability of healthcare services, such as cancer screenings, weight management, and mental health services.
~ Conducts public health campaigns on topics such as, immunizations, proper nutrition, or stress management.
~ Plans, develops, designs, and facilitates health education and training programs, including preparation, selection of educational materials/curricula, and follow-up activities.
~ Implements, monitors, and evaluates health education programs.
~ Documents activities, records health education information, and maintains health education databases.
~ Presents health education programs on an individual and group basis.
~ Works with and provides health education to a diverse group of communities.
~ Maintains current knowledge base on specific health issues, particularly in area of health education, health promotion, and community health.
~ Provides community outreach to promote and strengthen liaison relationships.
~ Serves on appropriate committees and task forces related to project.
~ Assists in completing written progress reports on a periodic basis.
~ Other duties as assigned.

~ Supervision is received from the Community Health Program Manager

~ No functional and administrative supervision is exercised over support staff

~ Bachelor"s degree in Community Health, Health Education, Health Promotion, or Public Health is required.
~ Must have current driver's license valid in the State of Iowa and be insurable under the County's guidelines.
~ Experience in working with the school aged and adolescent population. Knowledge, Skills and Abilities
Skills and Abilities - ~ Thorough knowledge of the principles, practices, and techniques of health education.
~ Thorough knowledge of social determinants of health and health equity.
~ Must be knowledgeable of local, state, and federal laws, rules, and regulations as they apply to the program area.
~ In-depth knowledge of local communities and community resources.
~ Must be proficient in all Microsoft Windows and other computer software applications.
~ Excellent written and verbal communication skills are required.
~ Multilingual skills preferred.
~ Ability to develop and maintain effective relationships with community, stakeholders, clients, and co-workers.
~ Ability to interact with persons from varied socioeconomic and ethnic backgrounds. Work Environment and Physical Demands
