Job Description:

Pay: $15.39 hourly
Work Schedule: M -F, 8a-4:30p

Provide goal-directed, medically necessary services and support to adults who have mental illness and substance abuse problems contributing to repeat hospital admissions, criminal justice system and crisis contact. Provide intensive case management to engage consumers, assist with access in community resources to address basic needs and to identify long term solutions in an effort to promote recovery. Regular, reliable, and non-disruptive attendance is an essential job duty, as is the ability to create and maintain collegial, harmonious working relationships with others. Must be able to complete complex tasks by applying standard office policies, authorized instructions, and past precedents to achieve a desired outcome. Must quickly recognize emergency or sensitive situations and take appropriate action. Critical decision making and the ability to think through the consequences of a decision are essential in this position.

Service Provision Case Management Services
  • Case Management using identified and accepted methods to remediate symptoms of mental illness and improve emotional and functional well-being
  • Conduct risk assessments as needed to address safety of clients
  • Provide Targeted Case Management for assigned consumers to aid in achieving goals identified in the individualized treatment plan; as: treatment planning, collateral contacts, phone support, access to supports and service coordination
  • Provide Community Psychiatric Supportive Treatment for assigned consumers to achieve treatment goals; interventions as: assistance in improving symptom self-management, strengths assessment and planning, assistance to assure consumer income maintenance, individual therapeutic intervention, medication education, crisis management, assistance in to achieve academic progress
  • Provide Attendant Care for assigned consumers to achieve treatment goals. AC interventions as: direct support, supervision and cueing
  • Provide outreach, screening and engagement services to potential participants
  • Arrange for the discharge of inpatient referrals and coordination of transition to the community
  • Provide transportation to support client attendance in medical appointments and other therapeutic goal related activities to include development of
community resources, employment or education access, social or consumer-run activities and related supports Treatment Planning
  • Prepare quality treatment plans in collaboration with client and/or guardian, additional treatment team and community partners
  • Complete DLA-20 at initial case management appointment and quarterly thereafter
Documentation
  • Complete Notes for Record, releases of information, safety documents and other documentation as required and/or recommended within established deadlines
  • Enters no show and cancellations for accuracy and completeness of medical record
  • Respond to Utilization Review staff feedback forms and make corrections as needed
  • Enter AIMS data monthly
  • Completes discharge summary when closing to case management services
  • Completes court reports or treatment summaries within required timelines
  • Ensure client’s financial information is current and up-to-date in medical record
Minimum Qualifications: Bachelor’s degree or equivalently qualified by work experience in the human services field and education, with one year of experience substituting for one year of education. Two years of post-High school education and equivalent combination of training or experience required. Must have a valid Kansas Driver’s license and current proof of automobile insurance. Meet the specifications as outlined in the CMHC licensing standards and pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens.
