We are currently seeking a full time In Homes Services Case Manager.
  • This position will be responsible for managing a case load of clients ranging in age from 4 to 18 with our contracting partner Tri-County Mental Health.
  • Duties may include treatment planning, linking with community resources, coordinating care with treatment team, and assisting families with positive coping skills.
  • Areas served include Clay, Ray, and Platte counties.

Bachelor's degree in Social work or related field along with two years of experience is required. Must have a clean driving record and applicable vehicle insurance.
